Required Forms and Fees

Each athlete MUST have a signed Drug and Alcohol Form, signed concussion form and a completed pre-participation physical form BEFORE they can start official practice on August 15.

The participation cost is a $90 fee for the first sport. Fees can be paid on line at HSD RevTrak:  Helena High School (revtrak.net)
